Pelvic Alignment, Birth, and Beyond: Why Support Before and After Baby Matters

I treat a wide variety of patients, but my true passion lies in treating the perinatal and pediatric population. Over the years, by far, the most repeated phrase I’ve heard from patients is  “I wish I knew about chiropractic with my first pregnancy”. I have made it my mission to get the word out so that it’s possible to be present and to enjoy the prenatal and the postpartum periods. 

Pregnancy and postpartum are two of the most physically transformative seasons a woman will ever experience. While so much focus is placed on the baby (rightfully so), the mother’s body is doing an incredible amount of work behind the scenes- and how well that body is supported can make a meaningful difference in comfort, function, and even birth outcomes.

Let’s talk about what that actually looks like!


Why Pelvic Alignment Matters During Pregnancy

As your baby grows, your body is constantly adapting. Due to hormonal changes, this happens pretty quickly after that second pink line appears. The hormone relaxin is responsible for loosening the ligaments, which helps your pelvis separate to deliver a baby. However, this ligament laxity creates more mobility in the joints and ligaments of the pelvis throughout the entire pregnancy. This is necessary for birth, but it also means the pelvis can become less stable and more prone to imbalance. Furthermore, muscles tense to stabilize your joints, which can cause discomfort.

When the pelvis isn’t moving and functioning well, it can show up in ways many women assume are “just part of pregnancy,” like:

  • Low back pain

  • Hip pain

  • Pubic symphysis discomfort

  • Round ligament pain

  • Pain rolling in bed or getting out of the car

  • Sciatic-type symptoms

  • A feeling of heaviness or asymmetry

These are common, but they aren’t something you should have to push through.

From a mechanical standpoint, the pelvis is the foundation for both your comfort and your baby’s positioning. When the pelvis is balanced and moving well, it creates more optimal space for your baby to settle into a head-down, well-aligned position.

When there are imbalances like sacral restriction, SI joint dysfunction, or ligament tension, it can contribute to:

  • Increased maternal discomfort

  • Reduced space for baby

  • Suboptimal fetal positioning


How Chiropractic Care Supports Pregnancy

Prenatal chiropractic care focuses on restoring balanced movement and alignment in the pelvis as well as addressing tension in the soft tissues.

This may include:

  • Gentle, pregnancy-safe adjustments

  • Addressing sacral and SI joint motion

  • Releasing tension in surrounding ligaments and muscles

  • Supporting nervous system regulation

The goal isn’t just pain relief (though that’s a big win). It’s helping your body function the way it’s designed to—so you can:

  • Move more comfortably day-to-day

  • Sleep better

  • Stay active longer into pregnancy

  • Create an environment that supports smoother, more efficient labor

When the pelvis is balanced and the nervous system is supported, labor often becomes more coordinated and efficient.


Postpartum: The Chapter No One Prepares You For

After birth, most of the attention shifts to the baby, but your body is still very much in recovery mode. 

Pregnancy and labor create:

  • Significant ligament strain

  • Core and pelvic floor disruption

  • Joint instability

  • Postural stress (feeding, holding, carrying baby)

And here’s something many moms don’t expect:

Symptoms often don’t show up right away.

In the first couple of weeks, adrenaline is high and activity is relatively low. But as you start moving more, lifting your baby more frequently, and settling into daily routines, things begin to surface. Additionally, relaxin doesn’t just leave the body when the baby does. It can stick around for up to a year after the baby is born (and longer if you’re nursing). Which means, baseline muscular tension is increased.

Common postpartum complaints include:

  • Mid-back and neck pain (especially with feeding)

  • Wrist and shoulder discomfort

  • Persistent pelvic or hip pain

  • Core weakness or instability

  • Lingering “pressure” in the pelvis


What Postpartum Chiropractic Support Looks Like

Postpartum care is about helping your body rebuild, not just recover.

This includes:

  • Restoring pelvic and spinal alignment

  • Supporting proper joint movement

  • Addressing compensations from pregnancy and birth

  • Improving nervous system regulation

  • Guiding safe return to activity

Care is always tailored to where you are whether that’s 2 weeks postpartum or 2 years.

Because the reality is: If something feels “off,” your body is asking for support.


Let’s Talk About C-Sections (April is C-Section Awareness Month)

C-sections are sometimes necessary and can absolutely be life-saving. Supporting moms through a cesarean birth is just as important as preparing for a vaginal one.

Preventative Support

While chiropractic care doesn’t “guarantee” any specific birth outcome, optimizing pelvic alignment and reducing tension can help support:

  • Better fetal positioning

  • More efficient labor progression

  • Reduced mechanical stress during labor

All of which may help lower the likelihood of certain avoidable interventions.

Even if you are planning for a c-section, we can work on balancing your pelvis and spine to keep you comfortable as your belly grows. Fetal positioning matters even when we know that the baby will be born via c-section.


Recovery After a C-Section

A cesarean birth is major abdominal surgery layered on top of pregnancy-related changes.

Recovery often involves:

  • Scar tissue formation

  • Core muscle disruption

  • Altered movement patterns

  • Compensation in the spine and pelvis

Many women experience:

  • Pulling or tightness around the scar

  • Back or hip pain

  • Difficulty reconnecting to their core

  • Postural strain from protecting the incision

Chiropractic care can support healing by:

  • Restoring alignment without stressing the incision

  • Reducing compensatory tension patterns

  • Supporting nervous system healing

The goal is to help you feel connected, stable and strong again.


Supporting the Mother Is Supporting the Baby

When a mother’s body is functioning well:

  • She can move, feed, and care for her baby more comfortably

  • Her nervous system is more regulated

  • Recovery is smoother and more sustainable

This isn’t about perfection—it’s about support.


Final Thoughts

Pain, imbalance, and dysfunction during pregnancy and postpartum are incredibly common, but they are also highly addressable.

By supporting pelvic alignment, nervous system function, and overall biomechanics, chiropractic care helps your body do what it was designed to do: adapt, recover, and thrive through every stage of motherhood.
